Insurance

Life Insurance is a benefit that provides a payment of a sum of money in the event of death of an eligible employee.
Accidental Death and Dismemberment (AD&D) Insurance provides benefits due to certain injuries or death from an accident.
•    A Basic Life and Accidental Death & Dismemberment (AD&D) Insurance Plan is provided to Full Time employees after 6 months of employment.
•    Supplemental Life Insurance is an employee paid benefit program offered to all Full Time and Part Time employees after 6 months of employment.
•    Supplemental Accidental Death & Dismemberment Insurance is an employee paid benefit program offered to all Full Time and Part Time employees after 6 months of employment.

Basic Life Insurance & AD&D is a benefit for Full Time employees paid by Redeemer Health. The benefit amount is one-time your base annual earnings with a minimum benefit of $50,000. The benefit begins the first day of the calendar month following six (6) months of employment.

Supplemental Life Insurance and Supplemental AD&D Insurance are employee paid benefits available to all Full Time and Part Time employees. The Supplemental Life and AD&D insurance allows employees to purchase additional life & AD&D insurance coverage for themselves, spouse, and/or dependent children. The benefits begin the first day of the calendar month following six (6) months of employment.

A Short Term Disability benefit pays you a portion of your earnings when you are out of work for a non-work related injury or illness. The weekly payments, up to the maximum benefit, will begin after the plan specific waiting period has been met. Redeemer Health has two different Short Term Disability plans as outlined above:
•    For employees who work in Pennsylvania, the Short Term Disability plan is an employee paid benefit available to all Full Time and Part Time employees. Employees are eligible for Short Term Disability coverage the first day of the month following three (3) months of employment. The coverage amount and premium is based on the employee salary and age.
•    For employees who work in New Jersey, employees are covered through the New Jersey State Disability Plan. Employees are eligible once they complete twenty (20) calendar weeks of employment meeting minimum weekly earnings.

A Long Term Disability benefit pays you a portion of your earnings when you are out of work for an injury or illness. The monthly benefit payment, up to a maximum benefit, will begin after the waiting period of 180 days.
•    Long Term Disability is a benefit for Full Time employees paid by Redeemer Health. The benefit begins the first day of the calendar month following six (6) months of employment.
•    Voluntary Long Term Disability is an employee paid benefit for Part Time Employees. The benefit begins the first day of the calendar month following six (6) months of employment.

Voluntary Hospital Care insurance provides benefit payments to the covered person for hospitalizations resulting from a covered injury or illness.
•    Employee paid benefit available to Full Time and Part Time employees.
•    Benefit starts first day of the month following six (6) months of employment

Voluntary Critical Illness Insurance pays a benefit directly to the covered person if they are diagnosed with a covered critical illness such as heart attack, cancer or stroke.
•    Employee paid benefit available to Full Time and Part Time employees
•    Benefit starts first day of the month following six (6) months of employment.
•    Benefits are payable for covered illnesses diagnosed after the effective date of coverage.

To complete your Supplemental Life Insurance/AD&D, Short Term Disability, Voluntary Long Term Disability Insurance, Voluntary Hospital Care or Voluntary Critical Illness Insurance you must create an account using ADP Health & Welfare benefits portal located on the intranet or https://my.adp.com. Click the “Sign Up” button and use the registration code: hrhs- welcome. Once you log in, review your coverage options and employee premiums and complete your enrollment. You have 30 days from your hire date to enroll as a new hire or annually during open enrollment.
